Organizing PCMH Quality Improvement Reporting—A New Tool

Practices have always been able to use NCQA’s Quality Measurement and Improvement Worksheet template to document quality improvement activities in the PCMH Survey Tool. The worksheet is optional for documenting the must-pass elements PCMH 6D and 6F (Implement Continuous Quality Improvement and Demonstrate Continuous Quality Improvement). Practices that use a template usually succeed in demonstrating requirements for maximum credit in these elements, and now the worksheet is even easier to use.

The next time you log into your survey license, you’ll find the new version of the worksheet, with step-by-step instructions for completing it, examples on how to complete each row of information (for a clinical measure and a disparity in care measure) and more space to enter requirements. The new format helps ensure that all required aspects are addressed for each measure reported. The worksheet can be added as a document to the survey and linked to PCMH 6D and 6F.

If you have already prepared the previous version of the worksheet, it’s fine to use it. Both versions provide the same reporting. You can use the new version in the future. If you have suggestions for improving other tools and worksheets, log into my.ncqa.org and send them to us!
