Patient-Centered Coordinated Care in Midlothian Village

NCQA traveled to Midlothian Village, Virginia, to get an “up-close and personal” perspective of how patient-centered coordinated care has transformed the quality of health care at Pediatric and Adolescent Health Partners, an NCQA-Recognized Patient-Centered Medical Home.
